Boca Nursing Services, Inc.’s Care Management Program is mission-driven to help the older adult remain safely in their chosen environment with an acceptable level of quality of life. Far too often, the quality of life diminishes as one ages when their loved ones are miles away. Moreover, it can be difficult to balance the responsibilities of helping an aging loved one and the responsibilities of your own family, career and daily life. Our Care Management Program will reduce the time, stress and additional costs often involved in caring for an older adult as they age.
Boca Nursing Services, Inc.’s Care Management Program will act as an advocate for your aging loved one and you. Our professional Care Managers are all Registered Nurses who work closely with healthcare providers, elder law attorneys, and a network of community resources to develop a plan of care to meet your loved one’s health, social, and safety needs. Components of the program include a comprehensive assessment (clinical, financial, social, psychosocial/cognitive abilities, home environment, transportation capabilities, etc.) and a plan of care designed to enhance health and care needs for quality living.
